UPHClean 1.6d is not needed for Windows 2008 or Windows VISTA, since those versions have a Profile service that is doing the same job.

The Ctx_SMAUser (SMA = Suite Monitoring and Alerting) local account is new with Presentation Server 4.x and generated during the installation. The account is most important and if not created correctly (set in different components like COM+, ICA listener etc.) a lot of things will not work. The Ctx_SMAUser is also the reason why you can't install Presentation Server on an Domain Conroller.
